Those who seek treatment abroad are often confronted with many questions: How will the stay be organised? How high will the treatment costs be? How will the doctor, the patient and their family communicate with each other? The Team Charité Health Care Service (CHS) is here to answer all these questions.

The following information is needed for a treatment request
Personal Information of the patient
- Name (surname/ first Name)
- Gender
- Date of birth
- Nationality
- Passport number
Photocopy of your passport
Please attach a digital copy of the patient’s passport to your email. The personal information listed above and both the photograph and signature must be clear and legible.

- Telephone number including Country and area codes
- Email address
Medical Condition
We would ask you to include a description of your current medical condition in as much detail as possible. Please let us know if you wish any specific or special types of treatment.
Medical Reports & Scans
Medical findings and images are only accepted in German and English. These must not be older than 6 months; oncological findings not older than 1 month. You have 3 options for transferring findings:
- By postal mail in print/pictures on CD
- By email as PDF files
- By a cloud solution. Please contact us for access data
